Opening or changing an X-ray facility in Oregon can feel confusing. This guide puts the state-level steps, forms, filing links, fees, and contacts in one place.

Step-by-step instructions
Step 1: Plan the room and use a licensed vendor
Complete the shielding design before construction or installation. The installer or vendor must hold the required Oregon license.
Step 2: Complete the registration application
Fill in the facility, owner, contact, practice, and equipment sections. List each tube and machine.
Step 3: Email or mail the form
Email the form to Judith.A.Smith@oha.oregon.gov, fax it to 971-673-0553, or mail it to Oregon Radiation Protection Services, 800 NE Oregon Street, Suite 640, Portland, OR 97232.
Step 4: Pay the invoice
Oregon reviews the form and sends an invoice. Pay it promptly. The state reports that certificates usually arrive about four to six weeks after payment.
Step 5: Keep the registration and update changes
Keep the certificate, shielding plan, installation record, and performance documents. Report ownership, address, and machine changes.
Cost
Current two-year facility fees include $427 for chiropractic, hospital, medical, osteopathic, and radiology facilities; $285 for industrial and podiatry; and $210 for academic, dental, and veterinary facilities.

Important reminder
