Step-by-step instructions
Step 1: Plan the room before construction
Submit plans for new X-ray construction so the lead requirements can be reviewed. South Dakota accepts a health physicist's shielding recommendations; Heartland Physics can prepare them.
Step 2: Create an account in the licensing system
Open the South Dakota healthcare licensing system and choose the X-ray facility license application.
Step 3: Complete the facility application
Enter the ownership, location, contact, room, and machine information. Upload requested construction, shielding, and safety records and pay the application charge shown by the system.
Step 4: Prepare the seven inspection record groups
Keep dosimetry reports, technique charts, processor or image-quality records, patient examination records, staff training, annual protective-device evaluations, and annual CT physicist reports when applicable.
Step 5: Renew annually and complete the three-year survey
Renew the facility license each year after December 1. Arrange a state or physicist X-ray survey at least every three years and correct any findings by the deadline.
