Step-by-step instructions
Step 1: Design the room
Finish the room layout and shielding design before construction. Keep the final shielding report with the facility records.
Step 2: Fill out Parts A, B, and C
Part A identifies the facility. Part B identifies the person responsible for radiation safety. Part C lists each machine. Use a separate line or continuation page for each unit.
Step 3: Attach machine and shielding records
Add the installer paperwork and any shielding report that applies to the project. Keep copies of equipment acceptance and performance tests.
Step 4: Mail the application
Send the signed package to Indiana Department of Health, Division of Radiology and Weights and Measures, 2 North Meridian Street, 4 Selig, Indianapolis, IN 46204.
Step 5: Do not operate until registration is complete
Keep the registration certificate, machine records, operating procedures, and required notices at the facility. Report later equipment or ownership changes to the state.
