A new facility files BRC-4. Each healing-arts X-ray machine gets its own BRC-4X. A new machine must be registered within 30 days after installation, and registration is not complete until the application fee is paid.

What to gather
- Legal facility name, physical address, mailing address, email, and phone
- Owner and Radiation Safety Officer information
- Machine type, manufacturer, model, serial number, tube count, maximum kVp, and installation date
- Installer information
- Scaled room plan and construction details
- Qualified-expert evaluation for CT or cone-beam CT when applicable
Step 1: Complete BRC-4
Mark New Facility. Enter the owner, facility, Radiation Safety Officer, machine count, facility type, and requested compliance information. The same form is used later for amendments and the 10-year renewal.
Step 2: Complete one BRC-4X for every machine
Enter the facility, machine type, manufacturer, model, serial number, tube information, room location, installation date, and operating use. Multiple machines require multiple BRC-4X forms.
Step 3: Attach the room and shielding information
For many stationary units and mobile units routinely used in one place, the technical package may include a scale drawing showing the room dimensions, machine, walls, doors, barrier construction and lead equivalence, nearby occupied areas, workload, and shielding calculations. Use a provider whose current qualifications cover the required work.
Step 4: Include equipment evaluations
For CT and cone-beam CT, include the latest or initial qualified-expert unit evaluation. The installer supplies installation and assembly records; Heartland Physics coordinates the physics portion with the facility.
Step 5: Calculate and include the fee
Arizona’s posted per-tube application and annual rates are $75 for hospitals; $51 for chiropractic, mammography, medical, and educational facilities; and $42 for dental, industrial, veterinary, and podiatry facilities.
The current state fee sheet says payment is not available online and the fee is not prorated.
Step 6: Send the new-facility package
Mail the signed BRC-4, all BRC-4X forms, required attachments, and payment to Arizona Department of Health Services, Bureau of Radiation Control, Attention: X-Ray Compliance, 4814 South 40th Street, Phoenix, AZ 85040.
Step 7: Keep the registration current
Send machine additions or removals on BRC-4X to XRAY@AZDHS.GOV. Amendments and renewals also use XRAY@AZDHS.GOV. Keep the registration, room plan, shielding calculations, unit evaluations, and installer records together.
Do not make this mistake
Do not send BRC-4 without a BRC-4X for every unit. Missing machine details, attachments, or payment delay the registration.
