Diagnostic medical X-ray is handled by OSDH; certain industrial, analytical, and therapeutic equipment is handled separately by Oklahoma DEQ.

Step 2: Complete the registration application

Step 3: Submit the application and fee
Submit the application and fee through the current OSDH licensing process. If the equipment is industrial, analytical, or therapeutic rather than diagnostic medical, confirm whether Oklahoma DEQ is the proper regulator before filing.
Step 4: Complete the post-filing requirements
Do not operate diagnostic X-ray equipment without the required permit. Retain the permit, shielding information, equipment inventory, inspections, calibrations, and applicable quality-control records.
Step 5: Renew and report changes
Oklahoma diagnostic X-ray permits renew annually.
